The Joint Commission Survey - Final Report

The Joint Commission Survey – Final Report

First of all, I want to say thank you to everyone for doing such a wonderful job of caring for our patients. During our final meeting with the surveyors from the Joint Commission, they told us that it was a privilege and honor to have spent a week in our facilities. They said they thoroughly enjoyed their time here.

The surveyors arrived on Monday and spent almost five full days looking at many of our processes, procedures and operations. At the end of the meeting this afternoon, they noted that they consider our list of recommendations to be unusually short.

The surveyors have 2,600 elements of performance that they look at. A facility in our category is allowed up to 13 issues of direct improvement. We had only five.

Here is what they found:
• Three surveyors spent a week here and found no observations of issues with hand hygiene.
• They found no observations related to treatment plans.
• There were no findings under national patient safety goals.
• The surveyors witnessed staff going way beyond just patient care by providing spiritual care and treating patients with dignity and respect.
• When they interviewed patients and families, the surveyors were told the families are delighted with the care they received here.
• The treatment planning is our behavioral health area is excellent.

Are their areas where we can improve? Of course there are. Here is what they noted.
• They recommended that we work to address the safety and security risks in pediatrics and mother-baby. The unit allows for potential unmonitored access by unauthorized persons.
• Certain rooms in the inpatient behavioral health unit could be improved from a suicide risk stand point.
• Primary source verification of licenses was not performed in a few cases.
• A supply room contained patient medical information. This issue was fixed while the surveyors were in the area.
• A surveyor identified the use of two prohibited abbreviations in an admission medication order.
• The doors to certain patient rooms in the inpatient behavioral health unit did not allow for proper egress.
• The pneumatic tube system has areas that are unsecured.
• Hospital logs did not document verification of frozen tissue temperature upon receipt.

Overall, I consider this a very successful review. This is a reflection of the great work that each and every one of you do every day. The surveyors had a discussion with our leadership team about the culture here. They wanted to know how we created such a wonderful environment.  Dr. Oram-Smith said,”The culture of caring of all those who work in our facilities is what sets us apart.”
I think that sums it up.

2010 Associate Campaign

2010 Associate Campaign - Going Above and Beyond

The sky is crystal blue with fluffy clouds and the sun is high and bright.  The balloon has been attached to the basket, inflated and the basket has taken flight in an exciting way… YOU DID IT!  The goal of $325,000 for the 2010 Associate Campaign, “Going Above and Beyond” has been met!  We are so glad that you have jumped in the basket to see the beauty that surrounds your generosity.  We are proud that the PSF associate participation rate is far above the national average – you send a strong message of support and encourage additional giving from external donors and funding sources. 

Now a challenge!  The Associate Campaign has solicited almost 3,000 VP’s, Directors, Managers, and Associates; however, the Foundation has only received half of the pledge forms given out.  Here’s the challenge: If you have not returned your pledge form, we challenge you to help us reach and exceed last year’s amount raised of $365,000!  It’s not too late to participate with a gift – YOU can push us over the goal even further and take us to even greater heights.  Remember, any gift is important and will provide crucial assistance to associates and patients in the coming fiscal year.  If you need another pledge form, please contact the Foundation.  

And, for those of you who have contributed every year for the past 10 years, we will soon be sending invitations to our Decade of Caring luncheon.  If you have contributed for the past 9 consecutive years, your gift this year will qualify you for this distinguished group and you will be invited to our luncheon at the end of June.

Hour Club gifts will be distributed on June 15 at Penrose Hospital and June 16 at St. Francis Medical Center.  We will have tables set up by the W elevators at Penrose Hospital and in the cafeteria at St. Francis Medical Center from 11 a.m.-3: p.m. and 6 - 9 p.m. Please mark your calendar to pick your gift up if you donated at the Hour Club level.

Flood Update


Since last Sunday’s water main break that flooded the basement of Penrose Hospital, crews have been working to clean up the damage. I am told that Blue Sky, the contractor in charge of the clean-up, removed most of the drying units Friday because the building was dry. It is estimated that complete repair will take more than a month as they will have to remove and replace a portion of the drywall that was wet. An industrial hygienist, in conjunction with Infection Control, has been monitoring the air quality in the basement throughout the week and will continue to do so during the repair process.

A few departments have had to move due to the damage.
• Medical Records has moved to the old Human Resources area in the basement. (North Elevators)
• On June 1, the Pharmacy is moving back to their temporary space on the 2nd floor while walls are replaced in their area of the basement.
• Radiology film storage has moved to the Gala Room. As was mentioned in an email yesterday, that room is unavailable for meeting or other events until further notice.
• As of right now, we plan for all other departments to stay in place while the repairs are made.

If you have questions or concerns about work processes between your department and any of the displaced departments, please contact the manager or supervisor of that department directly to address your concerns.

Thanks for your patience during this process. Again, a special thanks to everyone who worked extra on Sunday and the days following the flood to help with the clean-up and maintaining services for our patients.

Disaster Exercise

Disaster Exercise

To all associates:
On Saturday, June 5, Penrose St. Francis will be participating in a county wide Mass Causality Incident (MCI) full-scale exercise. The exercise will involve 17 local emergency response agencies in addition to the Pikes Peak American Red Cross, local Medical Reserve Corp., Penrose-St. Francis Health Services and Memorial Health System.

As a result of this exercise, we will be receiving actor/patients for the exercise who will be moulaged to represent traumatic injuries. These actor/patients will be brought into the emergency departments of both hospitals and then moved through the system to involve as many departments as possible in this exercise. The Joint Commission requires that we ‘stress’ our system at least once a year and this exercise will fulfill this requirement.

Please be prepared to explain to patients and visitors that some “patients” they may see in the halls are actors from our MCI exercise. Thank you in advance for your support during this annual exercise.

HLC Success Story

Great First Quarter and a Mad March

The Health Learning Center gym has had an outstanding first quarter. As usual, the HLC had an increase in numbers at the start of the new year, with New Year’s resolutions and people wanting to shed the few (or many) pounds they may have gained over the holidays. However, 2010 has been much better than the past few years. Through the SilverSneakers program, the HLC signed up 53 new members in January, 15 in February, 25 in March and 25 in April. We have had 24 new members sign up through our self pay program. That is 142 new clients already this year and averages out to about one new person a day! Needless to say, the increase in clients keeps the staff on their toes and moving.

The HLC gym also ran an exercise incentive called March Madness that ran in conjunction with the NCAA basketball tournament. The goal was to get folks to interact more and, of course, increase their exercise. We had 48 individuals participate. Clients were divided into 12 teams of four people.   Each player earned points for their team based on what type of exercise and how much exercise they did. Teams played each other for a week. At the end of the week, the team with the most points won. The top two teams from each division, as well as two wildcard teams, made the Championship Tournament. The bottom four teams played in a consolation tournament. The winner of the Championship Tournament was N-SHAPE. 2nd place was WildKats, 3rd place was FAB-4, and 4th place was A-Team.  The Consolation Tournament was won by the KosmoNots. The rest of the field included theCenturyOns,Cup O’ Moe, Freethrowers, Ketchikan’s Krew, Old Fudge, Rockettes and WiseGals.

To complete the incentive, there was an awards potluck. All participants were invited and received an award. The top three teams got to choose from a massage (thanks HIIP), King Soopers gift card or a pedometer. Overall, the incentive was a lot of fun and motivated our participants to increase their  exercise.

YOU CAN SAVE A LIFE...

YOU CAN SAVE A LIFE – MAYBE EVEN YOUR OWN!

Penrose Hospital and St. Francis Medical Center are Chest Pain Centers. This means that we are able to offer excellent care to the people with chest pain and heart attacks that come to our hospitals. It also means that we want to help our staff and our community to know when someone is having a heart attack and what to do when someone does. The information below will help you to know what to look for and what to do.

Heart disease is the biggest cause of death in the United States. Each year over a million Americans suffer a heart attack. Almost half of these are fatal. About half of people who die do so within an hour of the start of their symptoms and never even get to the hospital.

Fast action is the best weapon against a heart attack. We are able to perform procedures that can stop a heart attack in its tracks and lessen damage to the heart. The sooner they are performed, the more good they will do. Most people who have a heart attack wait too long – often two hours or more – to seek medical help. When someone waits, the risk of death or permanent damage  goes up. In a heart attack, heart muscle cells are dying and every minute matters.

Signs that someone may be having a heart attack. Some heart attacks are sudden and intense. Most start slowly, with mild pain or discomfort. Often people aren’t sure what’s wrong and wait too long before getting help. Here are some signs you might see in a patient, someone outside the hospital, or experience yourself that can mean a heart attack is happening:

• Chest Discomfort: most heart attacks involve discomfort in the center of the chest for more than a few minutes, or that goes away and comes back. In can feel like uncomfortable pressure, squeezing, fullness or pain.
• Discomfort in Other Areas of the Upper Body: including pain or discomfort in one or both arms, the back, neck, jaw or stomach.
• Shortness of Breath: with or without chest discomfort.
• Other Signs: may include breaking out in a cold sweat, nausea or lightheadedness.

Are the Symptoms Any Different for Women? Women’s most common heart attack symptom is chest pain or discomfort, but a third of women experience no chest pain at all. Women are more likely than men to experience other symptoms, like shortness of breath, nausea/vomiting, and back or jaw pain.

The bottom line:
• Learn and watch for the signs – remember they can be less obvious in women
• Act quickly! – even if you’re not sure, have it checked out. Ever minute matters!
• If you are in the hospital, notify a nurse or a doctor immediately
• If you are outside the hospital, call 911 – don’t let anyone drive the person to the hospital

National Health and Fitness Month


Helping HIIPster: “I love a challenge!”  Half marathon-check, triathlon training-check.
Sure, why not! Erin Tatum is always up for anything. Help HIIP out? – of course! Erin rallies her co-workers to participate in community walks and encourages all of them to volunteer at the annual HIIP health screening. She consistently volunteers at the HIIP health screening too. She inspires her co-workers with interdepartmental challenges and will fill in for them during part of their work shift which allows them to attend a HIIP Lunch N Learn or something fun like the Zumba demo class on National Employee Health and Fitness Day. She is a totally amazing and inspiring HIIP Rep and the HIIP department appreciates her energy, enthusiasm and helping spirit so very much!

The winner of the Helping HIIPster #3 trivia challenge is Barb Matzker! Congratulations!

Did you know?

We have three foot care nurses at the HLC who recently took a certification exam in order for our foot clinic to continue providing care. They had less than a month to prepare for a difficult exam and all three recently passed the exam. Congratulations on passing the exam so PSF can continue to provide outstanding foot care to our community.

Worse Punishment?
        An Air Force cargo plane was preparing for departure from Thule Air Base in Greenland. They were waiting for the truck to arrive to pump out the aircraft's sewage holding tank.
        The Aircraft Commander was in a hurry, the truck was late in arriving, and the Airman performing the job was extremely slow in getting the tank pumped out.
        When the commander berated the Airman for his slowness and promised punishment, the Airman responded: "Sir, I have no stripes, it is 20 below zero, I'm stationed in Greenland, and I am pumping sewage out of airplanes. Just what are you going to do to punish me?"

Military etiquette
        Officer: Soldier, do you have change for a dollar?
        Soldier: Sure, buddy.
        Officer: That's no way to address an officer! Now let's try it again. Do you have change for a dollar?
        Soldier: No, SIR!

Communication Breakdown...
        The reason the Army, Navy, Air Force, and Marines squabble among themselves is that they don't speak the same language. For example, take a simple phrase like, "Secure the building."
        • The Army will put guards around the place.
        • The Navy will turn out the lights and lock the doors.
        • The Air Force will take out a 5-year lease with an option to buy.
        • The Marines will kill everybody inside and make it a command post.

Hike for Hospice
On Sunday, June 27th, join us for the fifth annual Hike for Hospice to benefit Porter Hospice and St. Anthony Hospice. For more than twenty five years, these two non-profit hospice programs have helped thousands of people through their end-of-life journey, giving them comfort and peace at a time when they need it most. Hikers enjoy beautiful views as they climb to 7,600 feet to the top of Olinger Mount Lindo, home of the illuminated cross in Morrison.

All funds raised from Hike for Hospice go towards supporting charity care and hospice services not reimbursed by the Medicare Hospice Benefit including spiritual care, bereavement services, volunteer coordination and palliative care for children through the Butterfly Program.

Hikers can release a dove at the summit in honor or memory of a loved one while overlooking downtown Denver and the foothills. Leashed dogs are also welcome to hike.

T’ai Chi – A form of ancient Chinese martial arts that works to improve body awareness, strength and coordination.

Exercising with the Physioball Level II – For those individuals who have completed Level I, this class offers a more advanced and challenging full body workout. Metabolic Syndrome - Dr. Michael Koren, endocrinologist, along with dietitians, exercise physiologists and lifestyle counselors, will help you understand what metabolic syndrome is (a combination of risk factors obesity, high triglycerides, low HDL, high LDL and elevated blood pressure), what causes it and how you can reverse its deadly path.

Individual Fitness Assessment – Measure your fitness levels against the national averages and note areas in which you excel and potential areas of need. A sub-max VO2 treadmill test as well as evaluations for flexibility, strength and strength endurance will be performed.  Results will be reviewed and discussed with a fitness expert to help direct you in your future fitness goals.  Date/Time: By appointment only

Aquatics Classes – The Penrose Health Learning Center in cooperation with Colorado Sports and Spine. A structured class 2-3 times per week concentrating on range of motion, overall strengthening and general cardiovascular conditioning.  All Aquatics classes are located at 1625 Medical Center Point (Union and Templeton Gap).
